The investigation team determined that the general contractor of the project, Zhongnan Electric Power Design Institute Co., Ltd. was not strict in the review of the construction scheme, lacked effective control over the subcontracted construction units, and did not find and stop the illegal dismantling of molds and other behaviors of the project department of the construction unit.
The main problems of concrete suppliers
According to the identification of the investigation team, the direct cause of this accident is that the construction unit illegally dismantled the section 50 formwork in the case of insufficient concrete strength of the section 50 cylinder wall of the No. 7 cooling tower, resulting in the loss of formwork support of the section 50 cylinder wall concrete, which is not enough to withstand the upper load and collapses from the weak point at the bottom. Caused section 50 and above cylinder wall concrete and mold frame system to collapse continuously. The falling object impinges on the attached cable of the flat bridge connected to the inner side of the cylinder wall, resulting in the collapse of the flat bridge as a whole.

The production manager did not have the relevant knowledge and experience of concrete production, and the internal laboratory staffing did not meet the specified requirements. The key links of production are not strictly controlled, admixtures are not added in strict accordance with the concrete mix ratio, and concrete is supplied without pouring application.
